Good morning, Mr. Henderson. I see you came in today about some chest discomfort. Can you tell me more about what you have been experiencing?

Yeah, it started about three days ago. It is this pressure right in the middle of my chest. Sometimes it moves up to my jaw and my left arm gets kind of achy at the same time.

I see. On a scale of one to ten, how would you rate the severity? And does anything make it better or worse? Activity, rest, food?

Maybe a six out of ten when it hits. It seems to come on when I am walking up stairs or doing yard work. Sitting down and resting makes it go away in about five to ten minutes. Antacids did not help at all.
